·          AL - ALABAMA

o        Needs Insurance Certificate made out to: AL DOT, Permits Office, 1409 Coliseum Blvd, Montgomery, AL, 36130

o        Need Original Origin & Final Destination of move.

o        Need Load Make & Model               

·          AK – ALASKA

o        Need Original Origin & Final Destination of move.

o        Need Load Make & Model

·          AB – ALBERTA

o        Need axle Weights & Spacings               

o        Need tire sizes in metric.

o        Need Drivers License info including birth date.

·          AR - ARKANSAS

o        Insurance Certificate Needed for MOBILE HOMES ONLY

o        Needs Insurance Certificate made out to: AHTD Permit Section, P.O. Box 2779 Little Rock, AR 72203

o        Need axle Weights & Spacings

o        Need serial number for load.

·          AZ – ARIZONA

o        Need Insurance company name

o        Need IRP# and IFTA# if not registered in AZ.

o        Need Load Weight

·          CA – CALIFORNIA

o        Need Beginning & Ending address or junction’s if starting or stopping in the state.

o        A maximum of 40 feet on the kingpin setting is allowed in CA.

o        Mobile Homes over 14 feet wide must have certificate of insurance.

·          CO – COLORADO      

o        Need a HUD# for Mobile Homes starting in CO.

·          CT – CONNECTICUT

o        Need axle Weights & Spacings.

o        Need LOAD,TRAILER, and KINGPIN Length.

·          DE - DELAWARE

o        Need axle Weights & Spacings.

o        Need Original Origin & Final Destination of move.

·          FL – FLORIDA            

o        Need starting address / ending address for moves in the FLORIDA KEYS.

·          GA - GEORGIA

o        Needs Insurance Certificate made out to: GA Dept of Transportation, Oversize Permit Unit, Attn: Insurance Renewal Unit, 935 E Confederate Ave, Bldg 24, Atlanta, GA 30316

·          ID – IDAHO

o        Need Trailer length

·          IL – ILLINOIS            

o        Need MAKE, MODEL, and SERIAL # when overweight.               

·          IN - INDIANA            

o        Need M-203 on file with state. Forms available from On The Move Permits.

o        Need axle Weights & Spacings.

·          IA – IOWA    

o        Must be registered for the weight you’re hauling in IA.

o        For overweight loads, IA typically requests a copy of the CAB CARD to verify weight in IA.

·          KS – KANSAS            

o        Kansas’ maximum registered weight is 85,500.

·          KY – KENTUCKY      

o        Axle weights in groups.

o        Need KYU# or TEMP KYU# when load is over 60,000 lbs.

o        For Bulldozers over 12 ft wide: Does it have a bucket or blade?

·          LA – LOUISIANA                       

o        Need LA customer account #. Obtained by On The Move Permits for new customers.

o        For legal weight loads, how many days effective on the permit?

o        Must be registered for what you’re hauling in LA up to a maximum of 88,000 lbs.

o        Need insurance on file. This is done by the customer directly with LA.

·          ME – MAINE  

o        Need registered weight & gross weight.

o        For Mobile Homes, need YEAR, MAKE, SERIAL#, and COLOR of the home.

·          MB – MANITOBA      

o        Need IFTA#

o        Need Kingpin setting.

o        Need last PERMIT # for that customer.

·          MD – MARYLAND                      

o        Need weights of TRUCK, LOAD, and TRAILER when overweight.

o        Need MAKE, MODEL, and SERIAL # on most moves.

·          MA – MASSACHUSETTS       

o        No specific information is needed for MA, but the state charges for permits that are ordered whether they are issued or denied.

·          MI – MICHIGAN         

o        Need LENGTH, WIDTH, HEIGHT, and WEIGHT of load.

o        MI does not check for height clearance when over 13-6 high.

o        Max height for MOBILE HOMES is 15 ft high.

·          MN – MINNESOTA      

o        Need empty weight of tractor & trailer. Also need load weight.

o        For trailers over 53 ft long, they must be able to reduce down to legal size for permit to be issued.

·          MS – MISSISSIPI

o        Needs insurance certificate made out to: MDOT PO BOX 1850 Jackson, MS 39215

o        Need 6 digit customer ID # which must be obtained by customer once insurance is filed.

·          MO – MISSOURI        

o        Must file New Hauler form for new customers. Forms available from On The Move Permits.

o        Need insurance certificate made out to: MODOT Motor Carrier Services; 1320 Creek Trail Dr; PO Box 893; Jefferson City, MO 65102

o        Need MAKE, MODEL, and SERIAL # for most moves.

o        Is load being moved FOR HIRE or PRIVATE?

·          MT – MONTANA         

o        For new customers, need insurance company Name, Address, PH#, FAX#, Policy# ,Eff date, Exp date, Coverage amount.

o        Does customer have authority to move in MT? If not, must have Temporary registration and/or IFTA permits.

o        For MOBILE HOMES in MT, must have a transporter plate.

·          NE – NEBRASKA         

o        Must be registered for what you’re hauling in NE or the max allowed (94,000 lbs).               

o        Need insurance on file for MOBILE HOMES.

o        Need HUD # on all MOBILE HOME moves as well as the factories offline date.

·          NV – NEVADA            

·          NH - NEW HAMPSHIRE      

o        Routes submitted to NH must be working routes. NH does not route.

o        Most moves over 13-6 high require a route survey to be submitted.

·          NJ - NEW JERSEY

o        Needs insurance certificate made out to: NJ Division of Motor Vehicles, Special Permit Section, PO Box 133, Trenton, NJ 08666

o        Customers being added to NJ’s system for the first time can take 24-48 hours.

·          NM - NEW MEXICO            

o        Needs insurance certificate made out to: Shipper & Cosignee

·          NY - NEW YORK   

o        Needs axle Weights & Spacings.

o        Needs starting or ending address if starting or ending in the state.

o        Must be registered for the weight you’re hauling in NY.

o        Need Height survey for all moves over 13-6 high in NY.

o        No over height loads allowed on Thru-way.

o        NY Thru-way permits are valid for 1 day and must be submitted to the Thru-way authority by that day in order to move on the thruway 1 additional day.

o        NY City night moves is required when LOAD is over 10 ft wide. (10:00pm to 5:30am est).

·          NC - NORTH CAROLINA         

o        When an overweight move has a steer axle rating greater than 13,000 lbs, this is considered a Superload in NC. Allow 10 days for Superload approval.               

       

·          ND - NORTH DAKOTA            

o        Need axle Weights & Spacings.

o        Mobile Home moves need Address & Owner of the home when Starting or Ending in the state.

o        Needs HUD # for Mobile Home moves.

o        Need axle Weights & Spacings for all moves in ND.

o        What is registered weight in ND?

·          OH – OHIO    

o        Need OS-32 form on file with Ohio. Forms available from Ohio Dot’s website at: http://www.dot.state.oh.us/Divisions/HighwayOps/Maintenance/Permits/Pages/default.aspx               

o        Need starting & ending address or junctions when starting or ending in the state.

·          OK – OKLAHOMA     

o        Need starting & ending address or junctions when starting or ending in the state.

o        Need Serial # for most moves.

o        Need HUD # for used homes starting in OK.

o        Must be registered for what you’re hauling in OK. (90,000 lbs max)               

·          ON – ONTARIO            

o        Need CVOR # (This is the Canadian equivalent of the USDOT #)

o        Full Origin & Destination of move.

·          OR – OREGON            

o        Need Oregon File #. Obtained by customer by calling OR.

·          PA – PENNSYLVANIA

o        Need axle Weights & Spacings.

o        Need MAKE, MODEL, SERIAL #  of load.

·          QC – QUEBEC            

o        Need CVOR # (This is the Canadian equivalent of the USDOT #)

·          RI - RHODE ISLAND

o        Need original Origin & final Destination of move.

o        Need axle Weights & Spacings.

·          SK – SASKATCHEWAN         

o        Need CVOR # (This is the Canadian equivalent of the USDOT #)

·          SC - SOUTH CAROLINA         

o        Need DOT # always.

o        Need starting & ending address or junctions when starting or ending in the state.

o        Need axle Weights & Spacings.

·          SD - SOUTH DAKOTA            

o        For new customers, need insurance company Name, Address, PH#, FAX#, Policy# ,Eff date, Exp date, Coverage amount.

o        Need starting & ending address or junctions when starting or ending in the state.

o        Need axle Weights & Spacings.

·          TN – TENNESSEE

o        Need Kingpin setting. (Measurement in length from the pin of the trailer to the back of the trailer)

o        All Superloads must be approved by a supervisor before being issued.

·          TX – TEXAS  

o        Need TX customer account #.  Can be obtained by On The Move Permits for new customers.

o        For TX temporary registration permits, an insurance certificate must be submitted with the truck information listed.

·          UT – UTAH    

o        New customers must be added and approved to UT’s site online.

·          VT – VERMONT         

o        Need Original Origin & Final Destination of move.

o        Superloads require a certificate of insurance to be made out to: State of Vermont, Agency of Transportation, Dept of Motor Vehicles.

·          VA – VIRGINIA            

o        New customers must be added and approved to Virginia’s site online.

o        Need axle Weights & Spacings.

o        Need starting & ending address or junctions when starting or ending in the state.

·          WA – WASHINGTON

o        Night travel allowed up to 12 wide, 14-6 high and 105 ft long.

·          WV - WEST VIRGINIA            

o        Need driver name if towing a MOBILE HOME.

o        Canceled or denied permits are STILL CHARGED by WV whether the permit is issued or not.

·          WI – WISCONSIN     

o        All customers need a WI PC#. Applied for by On The Move Permits.

o        For buildings that come from a foundation: CDL driver's license # and a LC or MC# required.

·          WY – WYOMING         

o        All oversize and overweight permits must be obtained by customers calling their port of entry 150 miles prior to entering the state.
